Option 11 Stuck in INI loop
If an Option 11 PBX is caught in an INI loop and not booting, suspect corrupted files on the software daughterboard. If we're lucky, erasing the flash drive on the software daughterboard will correct the problem, without having to re-install the operating system and customer database.
Put the software distribution PCMCIA card in slot A, and power cycle the PBX to force it to boot from the card.
When you see the following:
*** To invoke install setup program from PCMCIA enter CONTROL-I ***
... enter CONTROL-B
This will invoke the MERIDIAN SMALL SYSTEM TEST MENU.
From this menu, we will select the FLASH TEST MENU.
From there we will select ERASE FLASH DRIVE ON DB.
When this process is complete, we will re-boot the PBX.
The sequence to delete the files on the software daughterboard is:

99 to re-boot PBX
The PBX should re-boot, rebuild the tempory files, and sucessfully load the customer database.
